Parent-Child Assistance Program (PCAP) Mentor
Whitecourt, Mayerthorpe, Lac St Anne & Woodlands County
This is a blended position based out of Whitecourt AB (35hrs/wk)

The Position:
The Fetal Alcohol Spectrum Disorder (FASD) Mentor provides in-home, one-on-one mentorship to individuals who may be prenatally exposed to alcohol. You will support the client to build and maintain a healthy lifestyle, including developing life and social skills, while supplying support, collaboration, and system navigation across multiple sectors.
The Parent-Child Assistance Program (PCAP) Mentor provides in-home, one-on-one mentorship services with a focus on reducing or stopping alcohol and/or drug use during pregnancy. You will work, utilizing a harm reduction approach and encourage healthy pregnancies, when applicable, and support overall wellbeing. You will also assist in the strengthening of caregiver-child relationships, promotion of healthy childhood growth and development outcomes, and encourage safe home environments.
You will have a role in enhancing the capacity of the community to prevent Fetal Alcohol Spectrum Disorder through awareness and prevention activities.
What you’ll do:

Travel to families’ homes in Whitecourt, Mayerthorpe, Lac St Anne County, Woodlands County
and surrounding areas to provide service

Establish trusting relationships with clients, their family, and support network
-
Assess strengths and needs in order to help create service plans and strategies to support the healthy development of the family/client
-
Work utilizing a strength based, relational, harm reduction approach
-
Utilize screens, assessments, and observation to guide service plan
-
Provide confidential, emotional, and practical support to parents and caregivers
-
Connect families/clients with other service providers and resources to improve their health and well-being
-
Advocate, as necessary, for clients to obtain the necessary supports for an improved quality of life, including basic needs, health, housing, transportation, legal, and income supports

Document all activities accurately and according to procedure
-
Transport clients to access services if necessary
Qualifications:
-
Compassionate with a positive and empathetic approach to others
-
Diploma in Social or Human Services, with relevant experience or equivalencies considered an asset
-
Experience working with individuals and families with complex needs
-
Knowledge and awareness of FASD considered an asset
-
Respect for the diversity of cultural differences and beliefs
-
Knowledge of community resources
-
Understanding of how to set professional boundaries
-
Ability to maintain computerized files and documentation
Employment requirements

Current clear Child Welfare Record Check and Criminal Record/Vulnerable Sector Check
-
Valid driver’s license with a reliable car will be required prior to start date
-
Ability to obtain vehicle insurance for business use with 2 million liability

FAMILY RESOURCE NETWORK HUB NAVIGATOR
Westlock
35 Hours per week

The Position:
You will be the first point of contact, providing program and resource information for agencies within the Rural Connections Family Resource Network. Responsible to forward referrals, coordinate services with community supports, and ensure that individuals and/or families do not get lost during the referral and connection process within the Family Resource Network. Responds to phone, email, and face-to-face inquiries. Pre-screening for program eligibility as required. Ensures efficient operation of the main office by supporting managers and employees by completing data entry, maintaining files, uploading documents into database, assisting with copying and scanning, and other administrative duties. Implements administrative systems, procedures, and policies.
What you’ll do:

Work collaboratively with Family Resource Network Spokes to lead meetings, collect/share information, (Facebook) etc.

Liaise with other Family Resource Network Hubs to share information, resources, and referrals.
-
Accept incoming program referrals, and/or intakes, and complete program participant screens.
-
Identify appropriate referrals to community supports and forward using the established Family Resource Network and Healthy Families Healthy Futures referral processes.
-
Maintain an up-to-date list of contacts, services, and resources, within the Family Resource Network
-
Create and maintain confidential, accurate, and current program participant records, including complete and thorough documentation for each program participant, in accordance with professional standards and internal policies.
-
Visit service agencies and attend community events to promote program referrals.
-
Co-Chair Westlock Interagency & participate in other interagency meetings and community events to increase program awareness.
-
Assist with reporting for program evaluation.
-
Update social media and website information on an on-going basis.
-
General reception duties including answering incoming calls and visitors in a friendly and courteous manner, directing callers to the appropriate person, and handling incoming and outgoing mail.

Travel to Smoky Lake, Thorhild, Waskatenau and Barrhead to promote FRN.
Qualifications:

Professional Diploma/Certificate, relevant experience and equivalencies considered an asset
-
Exceptional inter-personal skills with a positive and empathetic approach to others
-
Proficiency in Microsoft Office applications
-
Working knowledge of online databases
-
Respect for the diversity of cultural differences and beliefs
-
Knowledge of community resources
-
Adapt well to changes in work priorities as needed
-
Ability to maintain computerized files and documentation
-
Comfortable working independently
